Centers Home Care is actively looking to hire a Full-Time Care Coordinator to work for our Home Care Program.

* The Care Coordinator is responsible for delivery of outstanding home care services by serving as the primary point of contact for an assigned group of clients. * Primary duties include coordinating schedules for designated clients and caregivers, Schedule and coordinate 100% of assigned case load in an efficient, effective and timely manner, while ensuring outstanding customer satisfaction. * Monitoring day-of schedules and troubleshooting problems, and responding to client or caregiver issues as they arise. * Participate in overnight / on-call duties. * Maintain accurate schedules, clock-in/clock-out timecards, and clear notes for exceptions in the Company's systems, as required, to ensure accurate billing and payroll function.
* Good telephone, organizational, typing and communication skills necessary. Ability to maintain accurate schedules, notes and records. Must be able to complete overtime when needed. * Good interpersonal skills, team player.
